Account Deletion Request
If you use the S4Mobile application and wish to request the deletion of your account and associated personal data, you can do so by following the instructions below.
How to Request Deletion
To request deletion of your S4Mobile user account and personal details:
- By Email: Send an email to support@s4medical.co with the subject line "S4Mobile Account Deletion Request". Please send the request from the email address associated with your S4Mobile account.
- By Mail: Send your request in writing to:

Types of Data Deleted
Upon receiving and confirming your account deletion request, S4Medical will delete the following user data:
- Account credentials: Your login authentication profile and user permissions.
- Personal contact info: User name, professional email address, and phone number.
- App preferences: Local settings, login history, and device identification tokens.
Data Retention Policy (Protected Health Information)
Please Note: In compliance with federal and state medical records laws, HIPAA regulations, and our Business Associate Agreements (BAAs), any Protected Health Information (PHI) — including patient clinical notes, photos, and medical codes generated or documented under your account — belongs to the respective covered entity (your medical practice) and cannot be deleted upon user request. This data is archived and maintained securely in accordance with medical record retention requirements (typically 6–10 years depending on state laws).
Response Time
Once we receive your deletion request, we will verify your identity to protect your account's security. Account deletion requests are typically processed within 7 business days of verification, and we will send you a confirmation email once complete.
